Responsibilities*
  • This Advanced Practice Professional will provide health care and specialized health services to patients and families enrolled in the Cardiac Surgery Program in Cardiovascular Medicine at the CVC Clinic.
  • Under physician supervision, the Advanced Practice Professional will coordinate care of clinic patient's, perform consultations, pre-op evaluation and history, and follow up with the patients post-operatively.
  • Will also provide clinical expertise and direction to support patient medication management
  • Will collaborate with other care providers/consultants to coordinate and communicate clinical care
  • Maintain accurate documentation regarding patient status, evaluations, and treatments to physicians, the RN/Clinical Team; streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external Michigan Medicine team members and other ambulatory care providers
  • Participate in quality assurance activities including Lean Projects with the Cardiac Surgery clinical team as necessary.
  • Perform other related duties as required to assure patient safety and satisfaction
  • Participate in the development and presentation of educational experiences for patients, families and health care professionals.
  • Utilize Service Excellence and Patient Family Centered Care in daily practice
  • Abide by the standards of care and standards of practice established by Michigan Medicine
  • Practice within the regulations of the Michigan Public Health Code

Required Qualifications*
NP:
  • Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
  • Current specialty certification to practice as an Adult Gerontology Acute Care Nurse Practitioner OR Adult Gerontology Primary Care Nurse Practitioner.
  • Graduate of an accredited Nurse Practitioner program.
  • Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).
  • DEA registration required

PA:
  • Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
  • Certification by the National Commission on Certification for Physician Assistants (NCCPA).
  • Current license to practice as a Physician Assistant in the State of Michigan.
  • DEA registration required

Desired Qualifications*
  • 5 years' experience caring for the cardiac surgery population and/or cardiology patients
  • At least three year's working full time in an Advanced Practice role, preferably in cardiac surgery or Cardiology
  • Specific experience as an Advanced Practice Provider caring for patients with valve disease and treating hemodynamics.
  • At least 2 years' experience caring for chronic cardiology disease.
  • Demonstrated knowledge of adult cardiac physiology.
  • ACLS certified with-in 6 months of hire
  • Interprets telemetry and basic ECG.
  • Demonstrated ability to anticipate the clinical care needs of complex patients.
  • Must demonstrate a high level of independent problem solving and decision making abilities.
  • Demonstrated expert analytical skills and ability to prioritize assignments and manage busy work environment
  • Ability to demonstrate good computer skills including computer documentation and order entry.
  • Demonstrated clinical leadership and experience with coordinating and managing complex, patients and providing safe hand offs to the next provider.
  • Demonstrated expertise in facilitating the educational growth of others through previous clinical instructional role

Work Schedule
Hours/Week: 40 hours
Day shift - No on-Call /4 days.
Location: CVC Clinic
