Community Health Workers a frontline public health professional who act as a trusted bridge
between community members and healthcare or social service systems . School of Medicine CHW
who serves as a preceptor in student-led clinics, guiding students in patient-centered care and
community engagement. The Clinical CHW also supports and demonstrates effective care
coordination and resource navigation for patients, contributes to interprofessional team-based
care, and helps integrate community context and social needs into clinical decision-making.
Operations Program Managers
This position is patient-sensitive and must fulfill all associated vaccination requirements, or be approved for an exemption, prior to the first day of work. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.

Define and oversee a list of dependent projects needed to reach the program's overall goals. Enlist teams, implement strategies, and measure return on investment. Act as liaison between various departments, facilitating cross-team collaboration and understanding. Organize and track projects, proactively manage risks, manage project escalations, prioritize tasks, and meet specific business objectives.

Responsibilities
Role Specific:Teaching Responsibilities* The CHW will share knowledge with medical students on their experience working with the student-led clinic communities provide practical examples of health disparities and illustrate how social determinants of health influence healthcare decisions. * Guide and teach medical students to assess patients' needs for housing, food, transportation, and other social services and help by providing the necessary support.* Will be instrumental in providing clinical support and teaching students how to effectively communicate with patients from diverse backgrounds in the student-led clinic environment.* Use cases from the student-led clinic as case studies for students to study, and to be presented during intersession days. * Help medical students develop cultural humility by fostering an attitude of respect, curiosity, and self-reflection. Encouraging students to actively listen, learn from community members, and recognize their own biases and limitations. Through this experience, students will understand the importance of tailoring care to individual needs and preferences.Clinical Responsibilities* Provide health counseling and education to increase knowledge of self-sufficiency.* Help clients identify their health goals; and work closely with patients, students, and wellness coaches to help patients determine the best course of action for good health.* Provide patients with the care they need, CHWs work closely with an interprofessional team that includes doctors, nurses, students, and pharmacists.* Identify and help coordinate community events and workshops.* Participate in various community events helping community members with diabetes education and health needs.* Provide outreach to potential and existing clients in the community and at events.* Work with staff and volunteers to establish trusting relationships with clients while providing support and encouragement.* Will fill out the client's electronic intake questionnaire and screening assessment.* Will make regular follow-up calls and assist patients with follow-up appointments.* Collecting data on referrals, outcomes, and community impact (data/evaluation function)* Develop CBO partnerships, support health fairs and outreach events, conduct community needs assessments and listening sessions, and serve as a liaison between the SFESOM, clinics, and community partners.

Minimum Qualifications

Work Schedule Summary: Monday -- Friday, business hours. This position requires at least 24 hours/week presence onsite and is located at 1925 E Spence Eccles Drive Salt Lake City, UT 84132. May on occasion require 7:00 am virtual meeting participation.This is an in-person position, and it is directly related to the support of students in their experiential learning, and support of patients in the clinic.
